New Coordinated Assessment Resource The National Alliance to End Homelessness’ (the Alliance’s) Center for Capacity Building has released a new addition to its “Coordinated Assessment Toolkit: The Assessment and Referral Processes: Coordinated Assessment Checklist Addendum.

This resource serves as an in-depth guide to conducting assessments and referrals and is a companion to the “Coordinated Assessment Checklist.”

It includes a step-by-step process for:

  • Assessing and referring households and information on sharing data effectively through HMIS,
  • Incorporating consumer choice when making referral decisions, and
  • Creating opportunities for consumers to find their own housing while in shelter.

The document is based on the Alliance’s work with several communities around the country.
